Step 1: Visit www.mynexuzhealth.com and log in with your own details.

Choose 'CSAM' if you want to log in with your eID card or with 'itsme'.

Step 2: After this, click on 'create a link', and then the left box 'Do you want to access the medical records of others, e.g. your children?'.

Step 3: Then go to 'Children' and click on 'Parent/child link'.

Step 4: You will be redirected to our form where you can enter your child's details.

Be sure to double-check the information entered.

After entering the details, click on 'send'. Your request will be cancelled if incorrect details are provided.

Due to the high number of requests, this may take a few working days. Once the link has been properly made, you can access your child's record. In your medical record, click on your name and then on your child's name. This will enable you to switch between the different records.

Step 1: Open the 'mynexuzhealth' application and log in with your own data.

Step 2: Click on 'Accesses' at the bottom of the list.

Step 3: Go to 'Children'.

Step 4: Select ‘Parent/child link'.

Step 5: You will be redirected to our form where you can enter your child's details.

After entering the details, click on 'send'. Your request will be cancelled if incorrect details are provided.

Step 6: Click on 'send'. The form will be manually checked and processed, which may take a few days.

When you have a link to your child, click on your name and then on your child's name. This will enable you to switch between the different records.

Attention! Medical records of children under 15 are not available from all nexuzhealth partners via mynexuzhealth. As a parent, you can request a copy of the record from your hospital.
