How can I request access to my child’s health record (under 15 years)?

Parent/child link

You can request this linkage through either the mynexuzhealth.be website or the mynexuzhealth app. If additional information is needed, you’ll be notified. Once approved—this may take a few working days—you’ll be able to access your child’s medical record through your own account.
After approval, your child’s name will appear under your own in the platform, allowing you to switch between the records. Keep in mind: medical records of children under 15 are not available from all nexuzhealth partners. However, as a parent, you can always request a copy of the record directly from the hospital.

 

 

Requesting a parent-child link via the website

●    Step 1: Go to www.mynexuzhealth.be and log in using CSAM or itsme®.
●    Step 2: In the left-hand menu, select ‘Create a link’ and then ‘Do you want access to someone else’s medical record, such as your child’s?’.
●    Step 3: Under ‘Children’, select ‘Parent/Child Link’.
●    Step 4: Complete the form with your child’s information. Please double-check the details—incorrect data will result in a rejected request.
●    Step 5: Click ‘Submit’.

●    Step 1: Open the mynexuzhealth app and log in.
●    Step 2: Tap on ‘Access’ at the bottom, then choose ‘Children’.
●    Step 3: Select ‘Parent/Child Link’.
●    Step 4: Fill in your child’s details in the form.
●    Step 5: Submit the form. Please ensure the accuracy of the data—incorrect submissions will be denied.
