Representative

A patient can grant a mandate to a representative. This happens when the patient is no longer able to make decisions regarding healthcare.

Foster parent

Are you a foster parent? Then you may be able to consult the medical record of your foster child. Contact the ombuds service of your hospital for more information.

Legal guardian (administrator)

Is a person aged 18 or older (partially or fully) no longer able to take care of themselves? Then a legal guardian can be appointed to make, among other things, medical decisions.

Healthcare proxy holder

Within the hospital, you can be a healthcare proxy holder for another patient. You then make decisions about his or her medical care, within the limits of the mandate.
