How can I grant a trusted person access to my digital medical record?

A trusted representative can view your medical record, request a copy, and stay informed about your health status. You retain full control and can revoke access at any time via mynexuzhealth. Simply navigate to ‘Access’ > ‘Third Parties’ to disconnect them. You can also inform your treating physician that this person should no longer have access to your file.


You can grant access in three ways:

How do I grant access for a trusted representative via the website?

●    Step 1: Download the form ‘Assign a trusted representative’ [here].
●    Step 2: Fill it out and clearly indicate which rights you’re granting. Under ‘Period’, specify ‘(un)limited duration’ or ‘Until death’. For a limited period, provide an end date.
●    Step 3: Go to www.mynexuzhealth.com and log in.
●    Step 4: Click your name in the top-right corner, then choose ‘Access’.
●    Step 5: Under ‘Third Parties’, click ‘Third-Party Link’ to send a message.
●    Step 6: Upload the signed form as an attachment (drag and drop it into the message field).


Include the following information:
●    First and last name of your trusted representative
●    National identification number of your trusted representative


Once the link has been created, you’ll receive confirmation. The trusted person can then log into their own account and select your name to access your file.

How do I grant access for a trusted representative via the app?

●    Step 1: Download and complete the ‘Assign a trusted representative’ form.
●    Step 2: Indicate the rights granted and the duration, just as on the website.
   Step 3: Log into the mynexuzhealth app and go to ‘Messages’. Tap the pencil icon to create a new message.
●    Step 4: Choose your hospital and select ‘Patient Questions (Other)’. Tap ‘OK’.
●    Step 5: Attach the signed form using the paperclip icon and send the message.


Be sure to include the name, first name, and national identification number of your trusted person.

Once approved, the trusted representative will receive confirmation and can access your record by selecting your name in their own account.

How do I grant access for a trusted representative via the hospital’s ombuds service?

You may also complete the official government template and submit it to your hospital’s ombuds service.
