As a patient, can I authorise someone to access my record? (Third party access to records.)

A confidante may inquire about the health status, view the patient record, and request a copy of the patient record. The patient may undo access at any time via mynexuzhealth. Go to 'accesses', 'third parties', after which the patient may disconnect (revoke access) the confidante. The patient may also notify the professional (e.g. doctor) that the confidante is no longer authorised to act as initially specified.

A request may be made in three different ways:

Step 1: You can download the form ‘Appointing a confidante’ here.

Step 2: Complete the form in full. Tick or colour the boxes of the rights you assign to the confidante. Under 'Period' enter '(in)definite duration' or 'until death'. If the authorisation is for a "definite duration", complete it with an end date.

Step 3: Visit www.mynexuzhealth.be and log in with your personal details.

Step 4: Click on your name at the top right and then on 'Accesses'.

Step 5: Then go to 'third parties' and click on 'third party link' to send a message.

Step 6: As a final step, return the form completed and signed by the patient. You may add the document as an attachment by dragging it into the text field. Please add the following information in your request:

Name of confidante:

First name of confidante:

National register number of confidante:

You will receive a message as soon as the record is linked to your confidante. The confidante will log into their own record and will then click on their name to proceed to access the other patient's record. Click on the name of the record you want to access.

At any time, the patient can notify the professional (e.g. doctor) that the confidante may no longer act as initially specified.

Step 3: Log in to the mynexuzhealth app, then go to 'messages'. Then click on the pencil icon to create a new message.

Step 4: Select your hospital first, then check 'Patient questions (other)'. Then click on 'ok'.

Step 5: Return the form completed and signed by the patient. You can attach the form via the paperclip icon. Please add the following information in your request:

Name of confidante:

First name of confidante:

National register number of confidante:
