Which supporting documents may be required for parent-child linking?

If the address in the parent’s and child’s records doesn’t match, we cannot create the link immediately. To confirm parental authority, additional documents are required – this protects the patient’s rights under Belgian law.

You may provide one of the following:

●    Household composition certificate: Available via your municipality, or digitally with an eID via:
○    ‘My file’ (Rijksregister)
○    ‘My Citizen Profile’ (Flemish Government)
○    Your municipality’s e-desk

●    Divorce agreement (EOT): Official divorce document by mutual consent, requestable via the municipality where the divorce was recorded. Also available online with eID.

●    Birth certificate: Provide a copy via the parent/child request message. Available via your municipality’s e-desk or via ‘My Citizen Profile’ under ‘Certificates and permits’

●    Kids-ID: Belgian children under 12 receive a Kids-ID. Provide a photo of the front, clearly showing the parents’ names. More info is available at your local town hall
